Community Support Program
Adults with chronic mental illness frequently need assistance in meeting their needs and developing their potential without being unnecessarily isolated or excluded from the community. Persons who suffer from a severe and persistent mental illness that seriously impairs their functioning, may have deficits in such primary aspects of daily living as personal relations, living arrangements, and employment. These clients are served in our Day Treatment program, and also receive case management services and medication management as appropriate.
Programming is provided in four environments pertinent to their rehabilitation: living, learning, working, and socialization. To address these four areas, we employ specialists in vocational opportunities, housing and residential placement, treatment for residents of nursing facilities, and intensive case management.
